International Student Forms
Request to defer or switch the term you listed on your I-20.
An F-1 visa student may request a reduced course load if they meet immigration regulation requirements.
F-1 students who travel outside the U.S. must get a travel signature before leaving.
If you plan to transfer to 黑料社 from another college or school, you must complete the 黑料社 Transfer Intent Form.
Request to change your program. Requires DSO approval prior to change.
Medical Reduced Courseload Form
Take this letter to your medical professional to sign in order to qualify for medical course load reduction.
If you plan to drop or withdraw from 黑料社 before completing your program. You must complete this form.
If you will not complete your program by the date listed on your I-20, complete this form.
